Formatting Your Thesis: Appendices

Introduction
Appendices are supplemental to a thesis in nature and, when included, appear after the references/bibliography. If you are unsure about whether content should be included in the text of your thesis or in an appendix, consult with your supervisor.
If you wish to include an appendix or appendices in your thesis, include it/them within your thesis Word or LaTeX file.
Single space the appendix / appendices with spacing between paragraphs.
Materials included in appendices
Examples of material included in appendices are as follows--also refer to Formatting Help.
- interview questions
- participant letters / forms (sample appendix page)
- surveys / questionnaires (if not your own, these require copyright permission to be included)
- supplemental tables / figures / graphs / images
- extra-large (11" x 17" landscaped bi-fold) pages

CDs: audio files (.MP3s, example to come) and data files (.TXT, .XLS, see data file example)
- each disc must have its own Appendix page

- include an overall description of the subject matter, and any credits. If the disc contains more than one file, provide a list of filenames and a description of each file.

- include an overall description of the subject matter, and any credits. If the disc contains more than one file, provide a list of filenames and a description of each file. If a video is broken into multiple files, number the filenames according to the order in which they need to be viewed.
Note: Signatures, personal email addresses, and, personal phone numbers cannot be included in theses (on forms, letters, etc.). See sample of an appendix with blocked-out personal information.

Order of appendix(es)
- Appendix(es) appear in the order in which they are introduced in the text.
- CDs or DVDs appear as the last appendix.
Appendix headings
You may include one appendix or a number of appendices.
When only one appendix is included, type:
- the word "Appendix"
- a period
- 2 line-breaks ([Shift+Enter] twice)
- the appendix heading as follows.
Appendix.
Informed Consent Form
For more than one appendix, include a separate lettered appendix label and heading. Type:
- the word "Appendix" with the letter following
- a period
- 2 line-breaks ([Shift+Enter] twice)
- the appendix heading as follows.
Appendix A.
