SFU Library Fines and Penalties Appeal Form

Your appeal will be evaluated based on the reasons and information you have provided. It is important to explain all extenuating circumstances and, where possible, provide supporting documentation related to your case (by regular mail or in person) to: Access Services Division. WAC Bennett Library. 8888 University Drive. Burnaby BC V5A 1S6. or by fax to 778.782.3023 Note: Disagreements regarding the Library Loan Policy will not be considered as appeals.

While your appeal is being investigated, and in some instances this may take several weeks, your borrowing privileges will be reinstated; renewals and hold requests may be processed by staff at the Circulation Desk or by phoning 778.782.4345; no on-line transactions are available until your appeal has been resolved.

Online appeals are typically considered Monday to Friday 9 a.m. to 3 p.m. If you submit an appeal outside of working hours (late afternoons, holidays, weekends, etc.) your fines are suspended during the ensuing working days. Date of submission is taken into account as part of the appeals procedures.

In the event your appeal involves lost or material you believe you returned, and the material is not located during our investigation, you may be billed for processing charges, replacement costs, and/or any relevant overdue fines.

Please phone us at 778.782.4345 if you have any further questions.
